Maximum Dose of Pregabalin for Neuropathic Pain

Guideline Recommendations

  • The National Comprehensive Cancer Network recommends a maximum dose of 600 mg in divided doses three times daily for cancer-related neuropathic pain 1
  • European guidelines (ESMO-EONS-EANO) recommend a targeted dose of 300 mg twice daily (600 mg/day) for chemotherapy-induced peripheral neuropathy 2
  • Consensus recommendations for painful diabetic neuropathy support doses of 150-600 mg/day in divided doses 3

Dosing Considerations

  • Dose adjustment is mandatory for patients with creatinine clearance below 60 mL/min, as pregabalin is eliminated 98% by renal excretion 1

Adverse Effects

  • At 600 mg/day, expect somnolence in 15-25% and dizziness in 27-46% of patients 1
  • Peripheral edema occurs in approximately 10% of patients 3

Special Populations

  • Elderly or medically frail patients require slower titration and may not tolerate maximum doses 1
  • Patients with cardiac disease or those over 40 years should have ECG screening if combining pregabalin with tricyclic antidepressants 4
