Swollen Upper Eyelid Diagnosis and Management

Immediate Assessment and Diagnosis

  • The American Academy of Ophthalmology recommends checking for orbital involvement immediately by examining for proptosis, restricted or painful extraocular movements, vision changes, or severe pain, especially with eye movement 1, 2, 3, 4
  • The presence of red flags such as proptosis, restricted eye movements, or severe pain with eye movement indicates orbital cellulitis, requiring immediate CT scan with contrast and IV antibiotics, as per the American Academy of Pediatrics guidelines 1

Most Likely Diagnoses Without Conjunctivitis

  • Preseptal cellulitis presents with eyelid swelling, erythema, and warmth but without proptosis or impaired eye movements, and is treated with oral antibiotics covering Staphylococcus aureus and Streptococcus species, according to the Infectious Diseases Society of America guidelines 1, 5
  • Hordeolum (stye) is an acute, painful, localized swelling at the eyelid margin with focal tenderness, and is managed with warm compresses and gentle lid massage, as recommended by the American Academy of Ophthalmology 3
  • Chalazion is a firm, non-tender nodule within the tarsal plate, and is treated with warm compresses and observation for 4-6 weeks, with referral to ophthalmology if persistent or recurrent, as per the American Academy of Ophthalmology guidelines 6

Examination Technique

  • The American Academy of Ophthalmology recommends everting the upper eyelid to examine the tarsal conjunctiva for papillary hypertrophy or follicles, and performing fluorescein staining to rule out corneal involvement 5, 7, 1, 8

Treatment Algorithm

  • For preseptal cellulitis, the American Academy of Pediatrics recommends starting oral antibiotics, applying warm compresses, and following up within 24-48 hours to ensure no progression, with immediate referral if worsening or signs of orbital involvement develop 1, 3
  • For hordeolum or chalazion, the American Academy of Ophthalmology recommends warm compresses, gentle lid massage, and observation for 4-6 weeks, with referral to ophthalmology if persistent or recurrent 3, 6
