Management of Hemoptysis

Dosage and Administration

  • The maximum recommended dose of tranexamic acid for hemoptysis due to tuberculosis is 1 g intravenously every 8 hours, with a total daily dose not exceeding 3 g 1
  • Initial loading dose of tranexamic acid is 1 g infused over 10 minutes 1, 2
  • Maintenance dose of tranexamic acid is 1 g infused over 8 hours 1, 2
  • Tranexamic acid should be administered within 3 hours of bleeding onset for maximum efficacy 1, 2
  • Pre-hospital administration of tranexamic acid should be considered when possible 1, 2

Safety and Monitoring

  • Monitor for potential side effects of tranexamic acid, including seizures, particularly with high doses 1
  • Monitor for thrombotic events, a potential side effect of tranexamic acid 3, 4
  • Monitor renal function, as dose adjustment may be needed in renal impairment 5

Concomitant Therapies

  • Local hemostatic measures should be employed alongside tranexamic acid 2
  • Consider additional management strategies, including correction of coagulopathy, treatment of underlying tuberculosis, and bronchoscopic interventions for persistent bleeding 2

Special Considerations

  • Caution should be exercised in patients with known thromboembolic disease
  • Dosing of tranexamic acid should be adjusted in patients with renal failure, as it is renally excreted 5
