TB Skin Test Reading Time

Optimal Reading Window

  • The American Thoracic Society and CDC guidelines recommend that the tuberculin skin test must be read between 48 and 72 hours after injection, when induration reaches its maximum size 1, 2, 3, 4
  • The standard reading window is 48-72 hours post-injection, as recommended by the American Thoracic Society and CDC guidelines 1, 2, 3, 4, 5

Physiologic Basis for Timing

  • The tuberculin reaction begins 5-6 hours after injection as a delayed-type hypersensitivity response 1, 2, 6, 7
  • Maximal induration occurs at 48-72 hours due to T-cell recruitment, lymphokine release, and local inflammatory cell accumulation 1, 2, 6, 7

Special Circumstances

  • In elderly patients or first-time test recipients, the reaction may not peak until after 72 hours, though this delayed response does not alter test interpretation 1, 2
  • Tests read after 72 hours tend to underestimate the true size of induration and should be avoided 1, 2
  • Immediate hypersensitivity reactions (appearing within hours) disappear by 24 hours and should not be confused with the delayed hypersensitivity reaction that represents true tuberculin sensitivity 1, 2, 6, 7

Critical Reading Technique

  • The American Thoracic Society recommends measuring only the transverse diameter of induration (perpendicular to the long axis of the forearm), not erythema 1, 2, 3, 4, 5
  • Record measurements in millimeters; document "0 mm" rather than "negative" when no induration is present 1, 2
  • Reading should be performed in good light with the forearm slightly flexed, using both inspection and palpation 1, 2

PPD Test Reading Guidelines

Standard Reading Window

  • The American Thoracic Society and CDC guidelines recommend reading the tuberculin skin test between 48-72 hours after injection, when the delayed-type hypersensitivity reaction reaches its maximum induration, to ensure accurate interpretation 8, 9, 10, 11
  • The Centers for Disease Control and Prevention guidelines state that the physiologic basis for this timing is that the delayed hypersensitivity reaction begins 5-6 hours after injection and reaches maximum induration at 48-72 hours due to T-cell recruitment and local inflammatory cell accumulation 8, 9

Clinical Implications at 80 Hours

  • A clearly positive result at 80 hours may still be valid, as positive reactions can persist beyond 72 hours, according to the American Thoracic Society and CDC guidelines 8, 9
  • If a patient presents at 80 hours for reading, the test should be documented as read outside the recommended window and consideration should be given to repeating the test with proper 48-72 hour follow-up, as recommended by the Centers for Disease Control and Prevention 8, 9
