Management of Suspected Staphylococcal Eye Infections

Antibiotic Selection

  • The American College of Physicians and the Infectious Diseases Society of America recommend dicloxacillin 500 mg four times daily as the preferred oral antibiotic for methicillin-susceptible Staph aureus (MSSA) infections, while trimethoprim-sulfamethoxazole (TMP-SMX) 1-2 double-strength tablets twice daily is the drug of choice if methicillin-resistant Staph aureus (MRSA) is suspected 1, 2
  • For suspected MSSA infections, the Infectious Diseases Society of America suggests first-line treatment with dicloxacillin 500 mg orally four times daily, with cephalexin 500 mg orally four times daily as an alternative 3
  • If penicillin allergic (anaphylactic/immediate hypersensitivity), clindamycin 300-450 mg three times daily is recommended for MSSA infections 1, 3
  • For suspected MRSA infections, the Infectious Diseases Society of America recommends first-line treatment with TMP-SMX 1-2 double-strength tablets (160-320/800-1600 mg) twice daily, with doxycycline 100 mg twice daily as a second-line option 1, 2, 4

Special Considerations

  • The Centers for Disease Control and Prevention advise against using clindamycin empirically for MRSA without susceptibility testing due to potential resistance 1, 4
  • The American Academy of Pediatrics recommends avoiding tetracyclines (doxycycline/minocycline) in children <8 years old 1, 3
  • The Infectious Diseases Society of America suggests avoiding TMP-SMX in the third trimester and tetracyclines throughout pregnancy, instead using beta-lactams (dicloxacillin, cephalexin) as first choice 1, 3

Escalation to Parenteral Therapy

  • The Infectious Diseases Society of America recommends switching to intravenous antibiotics if systemic signs of infection, rapid progression despite oral therapy, immunocompromised host, or concern for deeper infection are present 1, 2
  • For MRSA infections, the Infectious Diseases Society of America suggests vancomycin 30-60 mg/kg/day divided every 8-12 hours or linezolid 600 mg IV every 12 hours as IV options 1, 4, 6
