Management of Small Fiber Neuropathy

Diagnostic Evaluation

  • The American College of Cardiology recommends diagnosis of small fiber neuropathy through skin biopsy showing decreased epidermal nerve fiber density, which is the gold standard, and functional assessment tests including thermoregulatory sweat testing and quantitative sudomotor axon reflex test 2, 3
  • Conventional nerve conduction studies are typically normal in pure small fiber neuropathy, according to the American College of Cardiology 2

Pharmacological Management

  • The American College of Cardiology recommends anticonvulsants, such as pregabalin and gabapentin, as first-line treatments for neuropathic pain in small fiber neuropathy 1, 4
  • The American College of Cardiology also recommends antidepressants, such as duloxetine and tricyclic antidepressants, as first-line treatments for neuropathic pain in small fiber neuropathy 1, 4
  • Antihistamines with serotonin blockade properties and beta blockers may be beneficial in some patients with small fiber neuropathy, according to the Mayo Clinic Proceedings 1
  • The American College of Cardiology recommends increased salt/fluid intake, fludrocortisone, midodrine, droxidopa, and pyridostigmine for orthostatic hypotension management in small fiber neuropathy 4

Disease-Modifying Treatments

  • The American College of Cardiology recommends TTR silencers, such as patisiran, vutrisiran, and inotersen, for hereditary transthyretin amyloidosis with polyneuropathy 4

Non-pharmacological Management

  • The Mayo Clinic Proceedings recommends pain rehabilitation programs with physical and occupational therapy for physical reconditioning and improved activity tolerance in small fiber neuropathy 1
  • Patient education on avoiding triggers while maintaining normal activities as much as possible is recommended, according to the Mayo Clinic Proceedings 5
  • Avoidance of extreme cooling methods, such as ice or cold water immersion, which may worsen tissue damage, is recommended by the Mayo Clinic Proceedings 3, 6

Special Considerations

  • Early diagnosis and treatment is crucial, as patients treated earlier have better outcomes, according to the American College of Cardiology 4
  • Regular monitoring of symptom progression using validated tools is recommended by the American College of Cardiology 2
  • For patients with refractory small fiber neuropathy, multidisciplinary pain rehabilitation programs should be considered, according to the Mayo Clinic Proceedings 1
