Propranolol for Anxiety: Efficacy and Clinical Applications

Current Evidence and Recommendations

  • The Canadian Clinical Practice Guidelines do not recommend beta blockers like propranolol as a first-line treatment for social anxiety disorder due to negative evidence 3, 1
  • SSRIs and SNRIs remain the first-line pharmacological treatments for most anxiety disorders with stronger evidence for efficacy 4
  • Propranolol may be useful for specific anxiety presentations, such as performance anxiety and specific social phobias, according to the Hypertension journal 2

Mechanism and Clinical Applications

  • Propranolol blocks beta-adrenergic receptors, reducing physical symptoms of anxiety such as tremor and nervousness related to sympathetic activation 5, 6

Alternative Approaches

  • The American College of Cardiology is not mentioned, however, first-line treatments for anxiety disorders include SSRIs and SNRIs, which have the strongest evidence 3, 4
  • Cognitive behavioral therapy (CBT) is also a first-line treatment for anxiety disorders, as recommended by the clinical guidelines 7
