Use of Omeprazole with Oral Anticoagulants

Indications for the Use of Omeprazole with Anticoagulants

  • The American College of Cardiology recommends that patients taking oral anticoagulants and having risk factors for gastrointestinal bleeding should receive gastric protection with proton pump inhibitors during the entire duration of anticoagulant therapy 2
  • Patients with a high risk of gastrointestinal bleeding who are taking oral anticoagulants should receive proton pump inhibitors, such as omeprazole, to reduce the risk of bleeding 1

Risk Factors Justifying the Use of Omeprazole

  • The American College of Cardiology and the European Heart Society suggest that patients with oral anticoagulants who are older than 75 years, have a history of gastrointestinal bleeding or peptic disease, or are taking multiple antithrombotic medications, should receive omeprazole 1, 2, 3, 4
  • Patients with oral anticoagulants who consume high amounts of alcohol, use chronic steroids or non-steroidal anti-inflammatory drugs (NSAIDs), or have a history of gastric or duodenal ulcers, should also receive omeprazole 2, 4, 5, 6

Efficacy of Proton Pump Inhibitors

  • Proton pump inhibitors are more effective than H2 antagonists, such as ranitidine, in preventing upper gastrointestinal bleeding in patients with antithrombotic therapy 7
  • Proton pump inhibitors reduce the risk of gastrointestinal bleeding in patients taking aspirin, dual antiplatelet therapy, or oral anticoagulants 1
  • In patients with high risk of bleeding, omeprazole has been shown to be more effective than H. pylori eradication in preventing the recurrence of bleeding ulcers 4

Considerations on Medication Interactions

  • Although there is a theoretical interaction between omeprazole and clopidogrel due to the use of the CYP2C19 enzyme, clinical studies have not demonstrated a significant increase in ischemic events when used together 1, 3
  • If there is concern about the interaction with clopidogrel, proton pump inhibitors that do not interfere with CYP2C19, such as pantoprazol, can be considered 5
