Prednisone Administration Timing

Dosing Recommendations

  • For single daily dose regimens, the European Society of Endocrinology recommends that prednisone should be taken upon awakening (before 9 am) 1
  • For divided dose regimens, the American College of Rheumatology suggests that the largest portion (typically 2/3) should be given in the morning and the remainder in early afternoon (not later than 4-6 hours before bedtime) 2, 3
  • Typical prednisolone dosing for adrenal insufficiency is 4-5 mg on awakening, or 3 mg on awakening and 1-2 mg at 14:00h, as recommended by the Endocrine Society 1

Benefits of Morning Administration

  • The American Academy of Sleep Medicine notes that morning administration of prednisone reduces the risk of insomnia, a common side effect when corticosteroids are taken in the evening 4, 2

Special Considerations

  • For patients experiencing morning nausea or lack of appetite, the European Society of Endocrinology suggests waking up earlier to take the first dose and then going back to sleep may help relieve symptoms 4, 1
  • Night shift workers should adjust their dosing schedule according to their sleep-wake pattern, as advised by the American College of Occupational and Environmental Medicine (e.g., taking the first dose upon awakening before going to work) 1
  • Patients with severe symptoms may benefit from divided doses, but the last dose should be taken no later than 4-6 hours before bedtime, as recommended by the American College of Rheumatology 3, 5

Monitoring and Adjustment

  • Clinical assessment is the primary method for monitoring glucocorticoid replacement adequacy, rather than laboratory values, according to the Endocrine Society 4
  • Signs of over-replacement include weight gain, insomnia, and peripheral edema, as noted by the American Heart Association 4, 2
  • Signs of under-replacement include lethargy, nausea, poor appetite, weight loss, and increased pigmentation, as reported by the European Society of Endocrinology 1, 2
  • Dose adjustments should be made based on patient symptoms, energy levels, and overall well-being, as recommended by the American College of Physicians 1, 3

Patient Education

  • The American College of Clinical Pharmacology advises that all patients on long-term prednisone should be educated on stress dosing for illness or injury 2
  • Patients should be aware of medication interactions that may affect prednisone metabolism, such as anticonvulsants increasing prednisone requirements, as noted by the European Society of Clinical Pharmacology 1
