Latest Target LDL Levels Based on Cardiovascular Risk
Risk-Based LDL Targets
- The American College of Cardiology recommends target LDL-C levels of <55 mg/dL for very high-risk patients, with a minimum 50% reduction from baseline 1
- Very high-risk patients, including those with established cardiovascular disease, diabetes with target organ damage, or severe chronic kidney disease, should aim for LDL-C <55 mg/dL and ≥50% reduction from baseline 1
- High-risk patients, including those with markedly elevated single risk factors, diabetes without target organ damage, or moderate chronic kidney disease, should aim for LDL-C <70 mg/dL and ≥50% reduction from baseline 1
- Moderate-risk patients should aim for LDL-C <100 mg/dL 1
- Low-risk patients should aim for LDL-C <116 mg/dL 1
Treatment Approach
- For very high-risk and high-risk patients, the American College of Cardiology recommends starting statin therapy immediately alongside lifestyle modifications to achieve ≥50% LDL-C reduction 1
- For moderate-risk patients, intensive lifestyle modifications should be started, with statin therapy added if targets are not achieved after 3-6 months, aiming for 30-49% LDL-C reduction 1
- For low-risk patients, lifestyle modifications should be the primary focus, with statin therapy considered only if LDL-C remains >190 mg/dL despite lifestyle changes 1
Clinical Benefits
- Every 1.0 mmol/L reduction in LDL-C is associated with a 20-25% reduction in cardiovascular events 1
