Treatment for Bile Acid Reflux
Diagnostic Considerations
- Bile acid reflux should be suspected in patients with risk factors, including a history of terminal ileal resection and abdominal radiotherapy, according to the American Gastroenterological Association 2
Treatment Algorithm
- The American College of Gastroenterology recommends cholestyramine as the first-line treatment for bile acid reflux, which forms an insoluble complex with bile acids in the intestine to prevent their reabsorption and reduce their irritating effects on the gastric mucosa 1
- High-potency Proton Pump Inhibitors (PPIs) such as esomeprazole or rabeprazole at 20-40 mg twice daily may help reduce overall gastric irritation, as suggested by the European Society of Gastrointestinal Endoscopy 3
Refractory Cases
- Oral sucralfate may be useful for recurrent bile reflux, as it forms a protective barrier on the gastric mucosa, according to the British Society of Gastroenterology 4
Monitoring and Follow-up
- Maintenance therapy should be given at the lowest effective dose, as recommended by the American Gastroenterological Association 5
- Assess symptomatic response to therapy, and if symptoms persist despite bile acid sequestrant therapy, consider alternative diagnoses or surgical options, according to the European Society of Gastrointestinal Endoscopy 4
