Management of Pulmonary Arterial Hypertension Crises
Immediate Recognition and ICU Admission
- The American Thoracic Society recommends immediate ICU admission for patients with PAH crisis, characterized by high heart rate, low systolic blood pressure, low urine output, and rising lactate levels, indicating imminent right ventricular failure 1
- The mortality rate for PAH patients admitted to ICU reaches 41%, making early recognition and aggressive intervention critical 1
Pharmacologic Management Algorithm
- The American College of Cardiology recommends initiating inhaled nitric oxide immediately as the preferred first-line pulmonary vasodilator, which decreases pulmonary vascular resistance and improves cardiac output 2, 1
- The European Society of Cardiology suggests starting dobutamine as the preferred inotrope for all hypotensive PAH crisis patients, as it has neutral or beneficial effects on pulmonary vascular resistance 2, 1, 3
- The American Heart Association recommends initiating continuous IV epoprostenol if the patient fails to stabilize on inhaled nitric oxide and dobutamine, with a starting dose of 2 ng/kg/min and increasing in increments of 2 ng/kg/min every 15 minutes or longer 1, 3
Respiratory Management
- The American Thoracic Society recommends maintaining oxygen saturation >90% at all times to prevent hypoxia-induced increases in pulmonary vascular resistance 2, 1, 3
- The European Respiratory Society suggests avoiding mechanical ventilation if possible, as positive intrathoracic pressure reduces venous return and worsens right ventricular failure, and employing a low tidal volume strategy if ventilation becomes necessary 3, 2, 1
Fluid Management
- The European Society of Cardiology recommends optimizing fluid balance with IV diuretics, as volume overload worsens right ventricular function, and avoiding aggressive volume expansion in patients with right ventricular failure 1, 3, 4, 5
Mechanical Circulatory Support
- The American College of Cardiology recommends initiating veno-arterial ECMO as a bridge to recovery or transplantation for patients with PAH crisis, low cardiac output, or right ventricular failure despite optimal medical therapy 2, 1
