Treatment of Bartonella Retinitis
Primary Treatment Regimen
- The CDC recommends a combination therapy of doxycycline plus rifampin for at least 3 months for Bartonella infections involving the eye and central nervous system, with doxycycline 100 mg twice daily PLUS rifampin 300-600 mg daily 1, 2
- This combination addresses both the infection and prevents recurrence, which is common with Bartonella, as recommended by the CDC and supported by the MMWR Recommendations and Reports 1, 3
Alternative Regimens
- For patients who cannot tolerate doxycycline, erythromycin (30-50 mg/kg body weight, maximum 2 g/day) is an acceptable first-line alternative, as recommended by the Praxis Medical Insights 1
- Azithromycin 500 mg daily plus rifampin 300-600 mg daily can be used as an alternative regimen, as recommended by the Praxis Medical Insights 2
Treatment Duration and Monitoring
- A minimum of 3 months of antibiotic therapy is recommended for ocular Bartonella infection, as recommended by the Praxis Medical Insights 1, 2
- Longer courses (4-6 months) may be necessary for complicated or persistent disease, as recommended by the Praxis Medical Insights 2
- Clinical assessment every 2-4 weeks to monitor visual improvement is recommended, as suggested by the Praxis Medical Insights 2
Ineffective Treatments to Avoid
- Penicillins and first-generation cephalosporins have no in vivo activity against Bartonella and should never be used, as recommended by the Praxis Medical Insights 1, 2
- TMP-SMX and quinolones as monotherapy have inconsistent clinical response and are not recommended, as recommended by the Praxis Medical Insights 1, 2
Prevention of Recurrence
- For immunocompromised patients, consider long-term suppressive therapy with doxycycline or a macrolide after completing initial treatment, as recommended by the MMWR Recommendations and Reports and the Praxis Medical Insights 1, 3
Pediatric Considerations
- For children with Bartonella neuroretinitis, doxycycline 2-4 mg/kg body weight (maximum 100-200 mg/day) divided into 1-2 doses is recommended, as suggested by the Praxis Medical Insights 1
