Treatment of Treatment-Resistant Depression and Anxiety

Rationale for SNRIs

  • The American College of Physicians suggests that SNRIs (duloxetine and venlafaxine) demonstrate superior efficacy for anxiety symptoms compared to SSRIs in patients with comorbid depression and anxiety 1, 2.
  • One trial showed venlafaxine had statistically significantly better response and remission rates than fluoxetine for patients with MDD and anxiety 1, 3.

Alternative Treatments

  • The American Psychiatric Association recommends considering mirtazapine as the next option if duloxetine fails, as it has a faster onset of action than SSRIs (within 1-2 weeks vs 4 weeks) 3, 4, 7.
  • Mirtazapine is superior to citalopram and paroxetine for early anxiety relief due to 5-HT2 receptor blockade 3, 4, 7.

Treatment Duration and Monitoring

  • The American Academy of Family Physicians suggests continuing treatment for at least 6-12 months after remission to prevent relapse 4.
  • Evaluate response at 4-6 weeks on target dose before declaring treatment failure 4.

Addressing Treatment Resistance

  • The National Institute of Mental Health states that 38% of patients don't respond to initial antidepressant trials and 54% don't achieve remission 4, 6, 7.
  • If duloxetine fails, consider augmentation with mirtazapine or atypical antipsychotic (aripiprazole, quetiapine) 5.
