Cardiomegaly Management and Treatment Approach

Diagnostic Evaluation

  • The European Society of Cardiology recommends that the management of cardiomegaly identified on chest X-ray should begin with a comprehensive diagnostic workup to determine the underlying cause, followed by targeted treatment of that specific etiology, as cardiomegaly itself is a radiographic finding rather than a disease entity 1, 2
  • Cardiomegaly on chest X-ray is defined as a cardiothoracic ratio >0.5 on PA films and >0.55 on AP films 2
  • Additional radiographic findings to look for include pulmonary venous congestion, pleural effusion, and interstitial or alveolar edema, which may suggest heart failure 1, 2
  • Echocardiography is the first-line confirmatory test to verify true cardiomegaly, assess ventricular size and function, identify structural abnormalities, and evaluate for valvular disease 1, 3
  • Electrocardiogram (ECG) is used to identify rhythm disturbances, chamber enlargement patterns, conduction abnormalities, and evidence of ischemia or infarction 1, 4, 5
  • Laboratory tests, including natriuretic peptides (BNP/NT-proBNP), complete blood count, renal function, electrolytes, and thyroid function tests, are essential for heart failure assessment and to rule out other causes of cardiomegaly 1

Treatment Approach

  • The European Society of Cardiology recommends pharmacological therapy, including ACE inhibitors or ARBs, beta-blockers, diuretics, mineralocorticoid receptor antagonists, and SGLT2 inhibitors, for heart failure management 1
  • Device therapy, such as cardiac resynchronization therapy and implantable cardioverter-defibrillator, is recommended for eligible patients with heart failure 1
  • Surgical repair or catheter-based interventions are recommended for structural defects causing volume overload, such as ventricular septal defect closure and patent ductus arteriosus closure 6, 7
  • Aggressive blood pressure control is recommended for patients with hypertension 1
  • Valve repair or replacement is recommended for patients with valvular disease 1
  • Revascularization is recommended for patients with ischemic cardiomyopathy 1
  • Disease-specific therapy is recommended for patients with infiltrative diseases, such as enzyme replacement for Pompe disease 5

Follow-up and Monitoring

  • Regular clinical assessment with attention to symptoms of heart failure is recommended 1
  • Serial echocardiography is recommended to monitor cardiac size and function 1
  • Adjustment of medical therapy is recommended based on clinical response 1
  • Follow-up intervals depend on specific lesions and residual abnormalities for patients with congenital heart disease 7
