Assessment and Management of Dyspnea in COPD
Introduction to mMRC Scale
- The European Respiratory Society recommends using the Modified Medical Research Council (mMRC) scale, a 5-point scale (0-4), to assess functional dyspnea in COPD management, with a score ≥2 indicating high symptom burden and increased risk for poor outcomes 1, 2, 3, 4
Scale Structure and Grading
- The mMRC dyspnea scale consists of five grades (0-4) that describe breathlessness severity based on functional limitations, with Grade 0 indicating no breathlessness except during strenuous exercise, and Grade 4 indicating breathlessness when dressing or undressing 1, 2, 3
Clinical Application in COPD
- The mMRC scale is recommended for assessing functional dyspnea in COPD management, with a score ≥2 indicating high symptom burden and increased risk for poor outcomes, according to the Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines 1, 2, 4
Risk Stratification
- An mMRC score ≥2 combined with GOLD grade 3 or 4 spirometry and/or frequent exacerbations (≥2 per year or ≥1 hospitalization) identifies patients at high risk for further exacerbations and poor clinical outcomes 1, 2, 4
- The mMRC is incorporated into multiple composite prognostic indices, including BODE, BODEx, ADO, and DOSE scores, to predict mortality and morbidity in COPD patients 1, 2, 5
Prognostic Value
- Increased mMRC levels are independently associated with increased mortality in COPD patients, according to the European Respiratory Society 2, 3, 6
