Management of Acute and Chronic Productive Cough in Children

Diagnosis and Duration

  • A productive cough lasting only 2 weeks is still considered acute, and the management approach differs significantly from chronic wet cough, which is defined as >4 weeks duration in children 1, 2

Treatment Approach

  • The American College of Chest Physicians recommends antibiotics targeting common respiratory bacteria, such as Streptococcus pneumoniae, Haemophilus influenzae, and Moraxella catarrhalis, for 2 weeks as the evidence-based treatment for chronic wet cough in children, with a Grade 1A recommendation 1, 2
  • Amoxicillin is the first-line choice at 45 mg/kg/day divided every 12 hours, according to the guideline society 3

Monitoring and Reassessment

  • If symptoms persist beyond 10 days without improvement, reassess for possible bacterial sinusitis or early protracted bacterial bronchitis, and consider initiating antibiotic treatment if cough continues to 4 weeks 3
  • If cough continues to 4 weeks, initiate the antibiotic protocol rather than continuing supportive care alone, as recommended by the guideline society 1, 2

Critical Warning Signs

  • High fever ≥39°C (102.2°F) requires immediate action, as it may indicate a bacterial infection 3
  • Respiratory distress, change in sputum color to yellow/green, paroxysmal cough with post-tussive vomiting or inspiratory "whoop", and specific cough pointers such as digital clubbing, coughing with feeding, failure to thrive, and hemoptysis are critical warning signs that require immediate attention and further investigation 1, 3, 4, 5
