Management of Severe Varicocele

Diagnostic Evaluation

• Scrotal Doppler ultrasound should be performed to confirm the grade of varicocele and evaluate blood flow patterns, particularly useful in patients where physical examination is difficult, such as obese patients 1, 2, 3
• Routine use of ultrasonography to identify non-palpable varicoceles is discouraged as treatment of these varicoceles is not helpful 1, 2

Special Considerations

• If the varicocele is on the right side or bilateral, no routine abdominal imaging is necessary unless it's a new onset or non-reducible varicocele 1, 3
