Dual Antiplatelet Therapy Guidelines for Acute Coronary Syndrome

Standard DAPT Duration by Clinical Scenario

  • The American College of Cardiology recommends continuing DAPT for at least 12 months after bare-metal stent or drug-eluting stent implantation in acute coronary syndrome patients with percutaneous coronary intervention (PCI) 2, 4
  • The European Heart Journal suggests that this applies to both STEMI and NSTE-ACS patients undergoing PCI 1, 2
  • After coronary artery bypass grafting (CABG) in ACS patients, the American College of Cardiology recommends resuming P2Y12 inhibitor therapy postoperatively to complete the full 12-month course 2, 4
  • For ACS patients managed without revascularization, the American College of Cardiology recommends continuing DAPT for at least 12 months 2, 4
  • The American College of Cardiology suggests using either clopidogrel or ticagrelor (ticagrelor preferred for NSTE-ACS) in patients managed without revascularization 2, 4
  • For STEMI patients receiving fibrinolytic therapy, the American College of Cardiology recommends a minimum of 14 days of clopidogrel (strong evidence), ideally extended to 12 months 2, 4
  • The European Heart Journal recommends using clopidogrel 300 mg loading dose in patients <75 years, then 75 mg daily 1
  • For stable coronary artery disease with elective PCI, the American College of Cardiology recommends a minimum of 6 months of DAPT after drug-eluting stent implantation 5, 2

P2Y12 Inhibitor Selection Algorithm

  • The European Heart Journal recommends ticagrelor as the preferred P2Y12 inhibitor for most ACS patients 1, 2
  • The American College of Cardiology suggests using prasugrel as a reasonable alternative for P2Y12 inhibitor-naïve patients undergoing PCI who have no contraindications 2, 4
  • The European Heart Journal recommends using clopidogrel when ticagrelor or prasugrel cannot be used 1, 3

Aspirin Dosing with DAPT

  • The European Heart Journal recommends maintaining aspirin at 75-100 mg daily when combined with any P2Y12 inhibitor 1, 5, 2, 4
  • The European Heart Journal suggests that higher aspirin doses increase bleeding risk without improving efficacy 1, 3

Modifying DAPT Duration Based on Bleeding Risk

  • The American College of Cardiology recommends considering shortening DAPT to 6 months in ACS patients with DES who develop high bleeding risk 2, 4
  • The European Heart Journal defines high bleeding risk as: PRECISE-DAPT score ≥25, need for oral anticoagulation, major intracranial surgery planned, or significant overt bleeding 1, 2, 3
  • The American College of Cardiology suggests considering extending DAPT beyond 12 months in ACS patients who tolerate therapy without bleeding complications 2, 4

Bleeding Risk Reduction Strategies

  • The European Heart Journal recommends implementing measures to reduce bleeding risk, including using radial arterial access for coronary angiography/PCI and prescribing a proton pump inhibitor (PPI) with DAPT 1, 3
  • The European Heart Journal suggests avoiding routine platelet function testing to adjust therapy before or after elective stenting 1

Perioperative Management

  • The European Heart Journal recommends not discontinuing DAPT within the first month after stent placement for elective non-cardiac surgery 1, 3
  • The European Heart Journal suggests continuing aspirin perioperatively if bleeding risk allows 1, 3
  • The European Heart Journal recommends resuming full DAPT as soon as possible postoperatively 1, 3

Critical Pitfalls to Avoid

  • The American College of Cardiology recommends never giving prasugrel to patients with prior stroke/TIA - this is a Class III: Harm recommendation with increased cerebrovascular event risk 2, 4, 3
  • The European Heart Journal recommends never stopping DAPT prematurely in the first month after stent placement, even for elective surgery 1, 3
  • The European Heart Journal suggests never using prasugrel in medically managed ACS (without PCI) - it is not recommended 1, 3
  • The European Heart Journal recommends never failing to switch from clopidogrel to ticagrelor in ACS patients when indicated and no contraindications exist 1, 3
  • The European Heart Journal suggests never forgetting to prescribe a PPI with DAPT to reduce GI bleeding risk 1, 3
  • The European Heart Journal recommends never using high-dose aspirin (>100 mg) with DAPT - keep it at 75-100 mg daily 1, 3
