Treatment of Iron Deficiency Anemia with Low Ferritin and Iron Levels

Diagnosis Confirmation

  • A ferritin level of less than 15 ng/mL confirms iron deficiency in a patient with anemia, as seen in a patient with a ferritin level of 8 ng/mL 1, 3
  • Transferrin saturation of 21% is borderline, but combined with low ferritin, strongly supports iron deficiency in patients with anemia 4
  • Depleted iron stores, as indicated by low ferritin and transferrin saturation, require supplementation to correct anemia and replenish iron stores 5

Treatment Approach

First-Line Therapy: Oral Iron

  • The American Gastroenterological Association recommends ferrous sulfate 200 mg three times daily as the first-line treatment for iron deficiency anemia due to its effectiveness, simplicity, and low cost 1, 5
  • Alternative oral preparations, such as ferrous gluconate and ferrous fumarate, are equally effective in treating iron deficiency anemia 5
  • Liquid preparations may be better tolerated than tablets if gastrointestinal side effects occur in patients taking oral iron supplements 5
  • Adding ascorbic acid (vitamin C) to oral iron supplements may enhance iron absorption in patients with poor response to treatment 5, 6

Dosing Considerations

  • Continue iron therapy for three months after correction of anemia to adequately replenish iron stores in patients with iron deficiency anemia 1, 3

When to Consider Intravenous Iron

  • Parenteral iron should be reserved for patients who are intolerant to at least two oral iron preparations 5
  • Parenteral iron may be considered for patients who are non-compliant with oral therapy 5

Monitoring Response

  • Hemoglobin concentration should rise by approximately 2 g/dL after 3-4 weeks of therapy in patients with iron deficiency anemia 5
  • Failure to respond to treatment suggests poor compliance, continued blood loss, or malabsorption in patients with iron deficiency anemia 5
  • Once normal, monitor hemoglobin and red cell indices every three months for one year, then after another year in patients with iron deficiency anemia 3
  • Additional oral iron should be given if hemoglobin or MCV falls below normal in patients with iron deficiency anemia 3
  • Consider ferritin estimation in doubtful cases of iron deficiency anemia 3

Investigating Underlying Causes

  • Heavy menstrual bleeding is a common cause of iron deficiency anemia in premenopausal women, affecting 5-10% of menstruating women 5, 7

Common Pitfalls to Avoid

  • Do not continue daily iron supplementation in the presence of normal or high ferritin values, as this is potentially harmful, although the exact citation is not provided
  • Do not overlook the need to treat for a full three months after anemia correction to replenish stores in patients with iron deficiency anemia 1
  • The goal of treatment is not just to correct anemia but to fully replenish iron stores to prevent recurrence in patients with iron deficiency anemia 1, 3
