Budesonide-Formoterol for COPD Management
Guideline Recommendations and Efficacy
- The American College of Chest Physicians and Canadian Thoracic Society guidelines strongly recommend maintenance combination inhaled corticosteroid/long-acting β-agonist therapy, such as budesonide-formoterol, compared with long-acting β-agonist monotherapy to prevent acute exacerbations in patients with stable moderate, severe, and very severe COPD (Grade 1C) 2, 3
- Combination ICS/LABA therapy is recommended over ICS monotherapy for preventing acute exacerbations of COPD (Grade 1B) 4, 2
- The 2023 Canadian Thoracic Society guideline notes that budesonide/formoterol has demonstrated efficacy in reducing exacerbation rates in high-risk COPD patients 1
- The benefit-risk ratio favors combination therapy with a number needed to treat of 4 patients for 1 year to prevent one moderate to severe exacerbation versus a number needed to harm of 33 patients for 1 year to cause one pneumonia 1
Patient Assessment and Therapy Consideration
- Budesonide-formoterol is particularly beneficial for patients with moderate to severe COPD (FEV1 <60% predicted) and a history of exacerbations 1, 2
- Consider budesonide-formoterol as maintenance therapy when patients have significant symptoms despite regular therapy with long-acting bronchodilators or have experienced repeated exacerbations 6, 4, 1
