Management of Elevated Levetiracetam Levels
Assessment and Immediate Management
- If the patient shows signs of respiratory depression, provide airway protection interventions and high-flow oxygen, according to the American College of Physicians, as recommended by a clinical oncology review 1
- Check blood glucose level to rule out hypoglycemia as a contributing factor, as suggested by a clinical oncology review 1
Seizure Management During Recovery
- If seizures occur during the recovery period, lorazepam 0.05-0.1 mg/kg IV (maximum 1-2 mg) can be administered, as recommended by a clinical oncology review 1
- For breakthrough seizures, alternative antiepileptic medications may be temporarily required, as suggested by the American College of Emergency Physicians 2
Resumption of Therapy
- Once levels have normalized and symptoms have resolved, consider resuming levetiracetam at a reduced dose with careful monitoring, as recommended by the American College of Emergency Physicians 2
Common Pitfalls and Caveats
- Avoid administering medications that cause CNS depression which could worsen symptoms, as warned by a clinical oncology review 1
- Do not abruptly discontinue levetiracetam long-term as this could precipitate withdrawal seizures, as cautioned by the American College of Emergency Physicians 2
Monitoring and Follow-up
- Monitor for seizure recurrence during the period of medication adjustment, as advised by the American College of Emergency Physicians 2
