Dark Red Scaly Rash on Lateral Legs: Diagnostic Features and Management

Key Diagnostic Features to Identify

  • The American Academy of Dermatology recommends looking for classic features of psoriasis, including well-demarcated erythematous plaques with silvery-white scale, and checking other body sites such as elbows, knees, scalp, presacral region, and nails for pitting or onycholysis 1, 2
  • Psoriasis typically presents with symmetrical distribution and may include pruritus, although it is not always severe, and a family history of psoriasis supports the diagnosis 1, 3

Diagnostic Approach and Management

  • The American Academy of Dermatology suggests inspecting the entire body for psoriatic plaques, nail changes, and scalp involvement, and examining feet for tinea pedis, which may indicate a dermatophyte infection 1, 2
  • For psoriasis, the American Academy of Dermatology recommends topical corticosteroids, vitamin D analogues, and referral to dermatology if the condition is extensive or refractory 2, 3
  • The American Academy of Dermatology also recommends treating tinea pedis concurrently if present to prevent recurrence of dermatophytosis 4

Referral and Escalation

  • The Annals of Oncology suggest referring patients with no improvement after 2-4 weeks of appropriate topical therapy, suspected psoriasis with more than 3% body surface area involvement, severe pruritus, diagnostic uncertainty, or signs of secondary infection 2, 4, 5
