Levofloxacin Dosing for Uncomplicated UTI

Introduction to Levofloxacin Use

  • The Infectious Diseases Society of America recommends levofloxacin 250 mg orally once daily for 3 days as an appropriate regimen for an adult with uncomplicated UTI and normal renal function, though fluoroquinolones should be reserved for cases where other first-line agents cannot be used 1
  • Fluoroquinolones, including levofloxacin, should be considered alternative antimicrobials for acute uncomplicated cystitis, not first-line therapy, due to their high efficacy but propensity for collateral damage 1
  • The IDSA guidelines explicitly recommend reserving fluoroquinolones for situations where other recommended agents cannot be used 1

Dosing Recommendations

  • For complicated UTI, levofloxacin 750 mg orally once daily for 5 days is an appropriate regimen for patients who are not severely ill 2, 3, 4, 5, 6, 7
  • For acute pyelonephritis, levofloxacin 750 mg orally once daily for 5 days or levofloxacin 500 mg twice daily for 7 days are both effective options 1, 8
  • The higher-dose, shorter-course regimen (750 mg for 5 days) has been shown to be non-inferior to ciprofloxacin 500 mg twice daily for 10 days, with clinical success rates of 81% and microbiologic eradication rates of 80% 2, 3, 4, 5, 6, 7

Resistance Patterns

  • Fluoroquinolones should only be used empirically when local resistance rates are known to be <10% 1, 8
  • If fluoroquinolone resistance exceeds 10% in your community, consider an initial dose of a long-acting parenteral agent before starting oral fluoroquinolone therapy 1, 8
