Gentamicin Dosage Guidelines

Introduction to Gentamicin Therapy

  • The American Heart Association recommends a gentamicin dosage of 3 mg/kg/day as a single daily dose for the treatment of streptococcal infections, with a duration of 2 weeks for penicillin-sensitive strains and 6 weeks for penicillin-resistant strains or prosthetic valve endocarditis 1, 2, 3
  • For staphylococcal infections, the recommended dosage is 3 mg/kg/day divided into 2-3 doses, with a duration of 2 weeks in combination with other antibiotics, and gentamicin should be administered in close temporal proximity with vancomycin, nafcillin, or oxacillin 4, 5, 6, 7, 8
  • The American Heart Association also recommends monitoring serum concentrations, with target peak concentrations of 4-6 mcg/ml and trough concentrations of <1-2 mcg/ml, and avoiding prolonged concentrations >12 mcg/ml 2, 3

Special Considerations

  • For patients with renal impairment, gentamicin should not be administered to those with a creatinine clearance <30 ml/min for the treatment of endocarditis, and the 2-week regimen is not recommended for patients with a creatinine clearance <20 ml/min 1, 2, 3
  • The dosage should be adjusted based on serum creatinine levels, with an interval between doses (in hours) = serum creatinine (mg/100 ml) × 8, or by reducing the dose by dividing the normal dose by the serum creatinine level 1, 2, 3

Endocarditis Treatment

  • The American Heart Association recommends a gentamicin dosage of 3 mg/kg/day as a single daily dose for the treatment of enterococcal endocarditis, with a duration of 6 weeks 4
  • For streptococcal endocarditis, the recommended dosage is 3 mg/kg/day as a single daily dose, with a duration of 2 weeks for penicillin-sensitive strains and 6 weeks for penicillin-resistant strains or prosthetic valve endocarditis 1, 2, 3
