Empiric Oral Antibiotic for Osteomyelitis with Purulent Drainage

Rationale for Antibiotic Selection

  • The Infectious Diseases Society of America recommends amoxicillin-clavulanic acid as the oral antibiotic to add to doxycycline for broadest coverage in this scenario, providing excellent coverage against both gram-positive organisms and gram-negative bacteria, as well as anaerobes that may be present in purulent drainage 1, 2, 3, 4
  • This combination is specifically recommended for complicated skin and soft tissue infections, particularly when broader coverage is needed beyond what doxycycline provides, with a strength of evidence considered high 2, 5

Microbiological Considerations

  • In osteomyelitis with purulent drainage, polymicrobial infection is common, with likely pathogens including Staphylococcus aureus (including MRSA), gram-negative bacteria, and anaerobic organisms in deeper infections 3, 4, 6, 7

Dosing Considerations

  • For moderate to severe infections such as osteomyelitis with purulent drainage, amoxicillin-clavulanic acid should be dosed at 875/125 mg twice daily, with doxycycline continued at 100 mg twice daily (already prescribed) 3, 4

Important Clinical Considerations

  • Purulent drainage suggests the need for surgical evaluation and possible debridement in addition to antibiotic therapy, with obtaining cultures of the purulent material essential before changing antibiotics if possible 4, 6, 8
  • Close follow-up is necessary to assess response to therapy, with consideration of broadening coverage further or surgical intervention if no improvement within 48-72 hours 2, 4, 6
