Diagnostic Approaches for Ruling Out Inflammatory Bowel Disease

Introduction to Diagnostic Tests

  • The American Gastroenterological Association recommends using fecal calprotectin as the primary screening test to rule out inflammatory bowel disease, with a threshold of 50-60 mg/g, due to its superior sensitivity (81%) and specificity (87%) 1, 2
  • Fecal calprotectin has a pooled sensitivity of 0.81 (95% CI, 0.75–0.86) and specificity of 0.87 (95% CI, 0.78–0.92) at the threshold of 50-60 mg/g, making it the most reliable non-invasive test for ruling out disease 1, 2

Comparative Test Performance

  • C-Reactive Protein (CRP) has a sensitivity of 0.73 (95% CI, 0.64–0.80) and specificity of 0.78 (95% CI, 0.58–0.91) at a threshold of 5-6 mg/L, but the AGA guidelines suggest against using CRP alone to screen for IBD due to suboptimal performance characteristics 4, 5, 3
  • Erythrocyte Sedimentation Rate (ESR) has a sensitivity range of 0.54 to 0.78, with specificity 0.46 to 0.95 at a cutoff of 10-15 mm/h, and the AGA explicitly recommends against using ESR to screen for IBD 4, 5, 1

Clinical Algorithm

  • The clinical algorithm for ruling out IBD involves ordering fecal calprotectin as the primary screening test, and if negative, considering functional bowel disorders, and if positive, proceeding to colonoscopy with biopsy for definitive diagnosis 1, 3, 2
  • If fecal calprotectin is unavailable or not covered by insurance, CRP may be considered as a compromise, but with lower sensitivity than fecal calprotectin 3, 4

Critical Considerations

  • Normal inflammatory markers do not exclude IBD, and clinical judgment and endoscopy remain essential when suspicion is high 6, 7
  • IgA deficiency can cause false-negative tissue transglutaminase results, and checking quantitative IgA level or using IgG-based tests is recommended if IgA-deficient 3
  • Fecal calprotectin can be elevated in other conditions, including infections, NSAIDs use, and microscopic colitis, and positive results require endoscopic confirmation 1, 3
