Tetanus Vaccination Guidelines for Wound Management

General Recommendations

  • The Centers for Disease Control and Prevention recommends that no additional tetanus toxoid is required for wound management if the patient received tetanus toxoid 2 months ago and has completed their primary vaccination series (≥3 doses), regardless of wound type 1, 2, 3
  • Patients who received their last dose within 10 years do not need tetanus toxoid for clean, minor wounds 1, 2
  • Patients who received their last dose within 5 years do not need tetanus toxoid for contaminated/tetanus-prone wounds 1, 2

Special Considerations

  • Patients with unknown vaccination history or fewer than 3 lifetime doses require both tetanus toxoid and tetanus immune globulin (TIG) for contaminated wounds 1, 2, 5
  • HIV-positive or severely immunocompromised patients with contaminated wounds should receive TIG regardless of vaccination timing 1, 2, 3
  • Patients with a history of Arthus reaction should not receive tetanus toxoid until >10 years after the most recent dose, even for contaminated wounds 1, 2

Immunologic Rationale

  • Complete primary vaccination provides long-lasting protection (≥10 years) 4
  • Antitoxin antibodies develop rapidly in persons who have received at least 2 doses 4
