Treatment Algorithm for Panic Disorder with Exam-Related Symptoms
First-Line Treatment: SSRI Monotherapy
- The American College of Cardiology is not applicable here, however, the recommended first-line treatment for panic disorder is an SSRI, such as sertraline 25-50 mg daily or escitalopram 5-10 mg daily, combined with cognitive behavioral therapy, with a short-term benzodiazepine considered for the first 2-4 weeks 1
- Sertraline is a preferred SSRI option, with a starting dose of 25 mg daily for 3-7 days, increasing to 50 mg daily by week 1-2, and a target dose of 50-175 mg daily 1
- Escitalopram is an alternative first-choice SSRI, with a starting dose of 5 mg daily for 3-7 days, increasing to 10 mg daily by week 1-2, and a target dose of 10-20 mg daily 1, 2
Adjunctive Short-Term Benzodiazepine
- A short-term benzodiazepine, such as alprazolam 0.25-0.5 mg as needed, may be added to bridge the gap before SSRI onset, but should not be used as monotherapy and should be discontinued after 2-4 weeks 1
Mandatory Cognitive Behavioral Therapy
- The American Psychological Association recommends immediate referral for individual cognitive behavioral therapy, targeting panic-specific cognitions and exposure to feared situations, with 12-20 individual sessions 1
- Cognitive behavioral therapy combined with an SSRI is superior to either alone 1, 3
Alternative SSRI if First Choice Fails
- If there is an inadequate response after 8-12 weeks at a therapeutic dose, an alternative SSRI, such as paroxetine or fluoxetine, may be considered 1, 2
Beta-Blockers: NOT Recommended for Panic Disorder
- The American Heart Association is not applicable here, however, beta-blockers, such as propranolol or atenolol, are not recommended for panic disorder treatment, as they do not treat the underlying condition and may provide only symptom relief for palpitations and tremors 4, 5
Monitoring Requirements
- Weekly monitoring for suicidal thinking is recommended, with a risk difference of 0.7% vs placebo, and a number needed to harm of 143 1
- Monitoring for serotonin syndrome, including mental status changes, neuromuscular hyperactivity, and autonomic instability, is also recommended 2
