Tenapanor Dosing and Administration
Introduction to Tenapanor Dosing
- The American Gastroenterological Association recommends that tenapanor should be administered at 50 mg twice daily, taken immediately before breakfast and immediately before dinner 1
Dosing Schedule
- The FDA-approved dosing regimen for tenapanor is 50 mg twice daily, with the morning dose taken immediately before breakfast and the evening dose taken immediately before dinner 2
Mechanism of Action
- Tenapanor works by inhibiting the sodium/hydrogen exchanger isoform 3 (NHE3) in the gastrointestinal tract, which decreases sodium and phosphate absorption in the intestinal lumen, increases water secretion into the gut, and has antinociceptive effects on abdominal pain 1, 2, 4
Evidence Supporting BID Dosing
- Twice-daily dosing produces superior pharmacodynamic effects compared to once-daily dosing with equivalent total daily doses, with 50 mg BID selected as the optimal dose 1, 5
- The 50 mg BID regimen achieved the FDA responder endpoint in 34.1% of patients versus 21.7% with placebo 1, 4
Clinical Efficacy
- Tenapanor demonstrated moderate certainty evidence for symptom relief (RR 0.84; 95% CI 0.79-0.90) 1, 5
- Improvement in abdominal pain (RR 0.81; 95% CI 0.73-0.88) 5, 6
- Improvement in complete spontaneous bowel movements (RR 0.83; 95% CI 0.77-0.90) 5
- 58.1% of patients reported adequate relief at 12 weeks versus 41.1% with placebo 5, 6
