Extended Home Infusion Treatment with Daptomycin and Zosyn for Severe Sepsis and Complicated Urinary Tract Infections

Clinical Rationale for Extended Therapy

  • The Infectious Diseases Society of America recommends 5-14 days of therapy for complicated urinary tract infections with Enterococcus, with longer durations (10-14 days) indicated for patients with severe sepsis and bloodstream infections 1, 3
  • The patient's complex clinical picture with multiple diagnoses, including urinary tract infection, severe sepsis, Enterococcus infection, deep vein thrombosis, and acute pyelonephritis, warrants extended therapy 1, 2

Antibiotic Selection Appropriateness

  • The American College of Clinical Pharmacy recommends daptomycin (6-12 mg/kg IV daily) for complicated urinary tract infections caused by Vancomycin-resistant Enterococci, with a treatment duration of 5-7 days for uncomplicated cases, but longer for complicated cases 1
  • For bloodstream infections with Enterococcus, the Society of Critical Care Medicine recommends daptomycin (8-12 mg/kg IV daily) with treatment duration of 10-14 days 1

Justification for Extended Duration

  • The Surviving Sepsis Campaign guidelines recommend that definitive treatment durations should be individualized according to infection sites, source control, underlying comorbidities, and initial response to therapy 3, 4, 5
  • For severe infections with sepsis, a treatment course of 10-14 days is suggested, according to the Infectious Diseases Society of America 3, 5

Medication-Specific Considerations

  • The Society of Critical Care Medicine recommends optimizing antibiotic dosing strategies based on pharmacokinetic/pharmacodynamic principles for severe infections like sepsis, and Zosyn (piperacillin/tazobactam) can be administered as extended infusion over several hours to increase time above MIC (T>MIC) and optimize efficacy 4, 6
