Tranexamic Acid Dosing for Post-Dental Procedure Gum Bleeding

Dosing Regimen

  • For gum bleeding control after a dental procedure in an adult with no significant medical history, the American College of Chest Physicians recommends using tranexamic acid 5% mouthwash (10 mL) 3-4 times daily for 1-2 days post-procedure 1
  • If prescribed prophylactically, one dose can be given 5-10 minutes before the dental procedure, as recommended by the American College of Chest Physicians 2, 1

Evidence Supporting Short Duration

  • The American College of Chest Physicians guidelines consistently recommend 1-2 days of post-procedure use across multiple iterations, with a strength of evidence supporting the effectiveness of short-duration regimens 2, 1

Preparation Instructions

  • The standard concentration of tranexamic acid solution is 5% (50 mg/mL), with a recommended volume per rinse of 10 mL (equivalent to 500 mg per rinse) 1

Important Clinical Considerations

  • The primary indication for tranexamic acid in dental procedures is for patients on anticoagulation therapy, such as those taking vitamin K antagonists (warfarin, other vitamin K antagonists), as recommended by the American College of Chest Physicians 2, 1
  • Patients with bleeding disorders or hereditary hemorrhagic telangiectasia may require tranexamic acid, according to the American Society of Hematology 3

Contraindications to Consider

  • Active thrombosis or recent thromboembolic event is an absolute contraindication for tranexamic acid use, as stated by the American Society of Hematology 3
  • Relative contraindications include atrial fibrillation or known thrombophilia, according to the American Society of Hematology 3

Tranexamic Acid Use in Dental Extractions

Indications for Tranexamic Acid

  • The American College of Chest Physicians recommends continuing vitamin K antagonists with coadministration of an oral prohemostatic agent, such as tranexamic acid mouthwash, for minor dental procedures in patients on anticoagulation therapy 4, 5.
  • Patients with normal coagulation not on anticoagulation do not require tranexamic acid for standard dental extractions, as there is no evidence supporting its routine use in this population 4, 5.

Contraindications and Cautions

  • Tranexamic acid is contraindicated in patients with active thrombosis or thromboembolism, and its use requires caution in patients with cardiovascular disease due to theoretical thrombotic concerns 6.
  • Approximately 90% of tranexamic acid is excreted unchanged in urine, and reduced doses are mandatory in patients with renal dysfunction due to increased risk of neurotoxicity and ocular toxicity 7, 8, 9, 10.

Administration Protocol

  • A dosing regimen of 10 mL of 5% tranexamic acid mouthwash solution 5-10 minutes before the dental procedure, then 2-4 times daily for 1-2 days post-procedure, is recommended for at-risk patients 4, 5.

Patient Assessment

  • The American College of Chest Physicians guidelines support assessing bleeding risk by evaluating if the patient is on anticoagulation, has a bleeding disorder, or has cirrhosis with severe coagulopathy 4, 7, 8.
  • Patients with cirrhosis and acceptable parameters (INR <2.50 and platelet counts >30 × 10⁹/L) may not require tranexamic acid, as soaking gauze with tranexamic acid did not impact outcomes in this population 7, 8, 9.
