Management of Moderate Mitral Regurgitation
Initial Diagnostic Confirmation
- The American College of Cardiology recommends obtaining comprehensive echocardiography to verify MR severity using multiple parameters: vena contracta width, EROA, regurgitant volume, and regurgitant fraction 3, 4
- Consider additional testing (TEE, CMR, or cardiac catheterization) when uncertainty exists between mild-to-moderate or moderate-to-severe MR 3, 4, 1
- Distinguish primary from secondary MR, as this fundamentally changes the management algorithm 1, 2
Management Algorithm for Moderate Primary MR
- The European Heart Journal guidelines state that surgery is NOT indicated for isolated moderate primary MR in asymptomatic patients, even with excellent valve repair feasibility 5, 6
- Multidisciplinary heart team evaluation is warranted to assess whether intervention is appropriate for symptomatic patients with moderate primary MR 5, 1, 2
- MV repair may be considered at the time of coronary artery bypass grafting for patients with multivessel coronary artery disease and moderate secondary MR 7, 8
Management Algorithm for Moderate Secondary MR
- The American College of Cardiology recommends Guideline-Directed Medical Therapy (GDMT) as first-line therapy, including ACE inhibitors or ARBs, beta-blockers, mineralocorticoid receptor antagonists, diuretics, and nitrates 1, 2
- Implement Cardiac Resynchronization Therapy (CRT) in patients meeting guideline criteria before considering any valve intervention 1, 2
- Monitor for progression to severe MR (EROA ≥0.3-0.4 cm²) and assess for development of symptoms despite optimal medical therapy 5, 1, 2
Critical Clinical Pearls
- Confirm MR severity with comprehensive multiparametric echocardiographic assessment before proceeding to intervention 3, 4
- Distinguish primary from secondary etiology and maximize GDMT for secondary MR 1, 2
- Multidisciplinary heart team discussion is necessary when intervention is being considered 5, 1, 2
