Medical Necessity of Anterior Cervical Discectomy and Fusion (ACDF) for Cervical Radiculopathy

Required Documentation for Medical Necessity Determination

  • The American College of Surgeons recommends that ACDF requires documentation of signs or symptoms of neural compression associated with the levels being treated, including radiculopathy and myelopathy, with moderate to severe central/lateral recess or foraminal stenosis, or nerve root/spinal cord compression at levels corresponding with clinical findings 1
  • Advanced imaging studies must show correlation between imaging findings and specific symptoms, with documentation of impact on activities of daily living 1

Conservative Management Requirements

  • Non-operative management should be the initial treatment for degenerative cervical disc disease, with 75-90% of patients achieving symptomatic improvement through conservative measures, including pharmacological treatment, physical therapy, and epidural steroid injections for radicular pain 2
  • At least 6 weeks of conservative management must be documented before surgical intervention, with a standard conservative approach including cervical strengthening and flexibility exercises, and medications such as acetaminophen and/or NSAIDs 1, 2

Surgical Indications When Criteria Are Met

  • When properly indicated, ACDF can achieve good to excellent outcomes in approximately 90% of patients, with anterior cervical plating recommended over ACDF without instrumentation to improve arm pain, based on Class II evidence 1, 3
  • Allograft materials are considered medically necessary for spinal fusions when other criteria for the procedure are met, with clear correlation between imaging findings and symptoms 4, 1
