Augmentin Dosage Guidelines

Introduction to Augmentin Dosage

  • The American College of Physicians and Infectious Diseases Society recommends administering Augmentin twice daily (BID) for most indications, with three times daily (TID) dosing reserved for specific severe infections or when using standard-dose formulations for high-risk pathogens 1

Infection-Specific Dosage Guidelines

  • For animal and human bite infections, the recommended regimen is Amoxicillin-clavulanate 875/125 mg BID, as suggested by the Infectious Diseases Society of America 1
  • For sinusitis, 875/125 mg BID for 5-7 days is the standard regimen, according to the Clinical Infectious Diseases guidelines 2
  • For erysipeloid, Amoxicillin 500 mg TID for 7-10 days is recommended, as per the Clinical Infectious Diseases guidelines 1
  • For severe infections with suspected resistant pathogens, TID dosing may be necessary to achieve higher drug concentrations, as recommended by the Infectious Diseases Society of America 1
  • For metronidazole-like dosing for anaerobic coverage, 500 mg TID is recommended when treating vertebral osteomyelitis due to anaerobes, according to the Clinical Infectious Diseases guidelines 3

Formulation and Bacterial Susceptibility Considerations

  • Extended-release formulations (Augmentin XR) are designed for BID dosing, as stated by the Clinical Infectious Diseases guidelines 1
  • For less susceptible organisms (MIC ≥4 mg/L), more frequent dosing may be needed, as recommended by the Infectious Diseases Society of America 1

Patient-Specific Considerations

  • Patients with impaired renal function may require dose adjustment but typically maintain the same frequency, according to the Clinical Infectious Diseases guidelines 1
  • BID dosing generally improves medication adherence compared to TID dosing, as suggested by the American College of Physicians 1

Potential Risks and Considerations

  • Underdosing may lead to treatment failure, while overdosing may increase side effects and reduce adherence, as warned by the Infectious Diseases Society of America 1
  • Confusion with other beta-lactams may occur, but newer Augmentin formulations are designed for BID administration, as stated by the Clinical Infectious Diseases guidelines 1
