Maximum Duration of Enoxaparin in Conservatively Managed NSTEMI
Evidence-Based Duration Guidelines
- The American College of Cardiology/American Heart Association (ACC/AHA) guidelines recommend continuing enoxaparin for the duration of hospitalization, up to 8 days, with a Level of Evidence: A 1, 2, 3, 4, 5, 6
- The ACC/AHA guidelines suggest discontinuing anticoagulant therapy after 8 days or at hospital discharge, whichever comes first, with a Level of Evidence: A 1, 2, 3, 4, 5, 6
Clinical Context and Application
- For patients who undergo angiography but remain on medical management, the ACC/AHA guidelines recommend continuing enoxaparin for the duration of hospitalization, up to 8 days, with a Level of Evidence: A 4, 5, 6
- For patients who do not undergo angiography or stress testing, the ACC/AHA guidelines recommend administering enoxaparin for the duration of hospitalization, up to 8 days, with a Level of Evidence: A 1, 2, 3, 4, 5, 6
Important Caveats and Pitfalls
- The 8-day maximum duration of enoxaparin is absolute for NSTEMI patients, and should not be confused with VTE prophylaxis guidelines, according to the ACC/AHA guidelines 1, 2, 3, 4
- The ACC/AHA guidelines recommend discontinuing anticoagulant therapy immediately after PCI for uncomplicated cases 1, 2, 3
- The ACC/AHA guidelines recommend discontinuing enoxaparin 12-24 hours before CABG surgery and transitioning to UFH 1, 2, 3, 9
