Documentation Requirements for Home Health Services for Weakness

Essential Medical Justification Elements

  • The American Thoracic Society recommends documenting at least one qualifying criterion to justify home health referral, including functional limitations requiring assistance with activities of daily living (ADLs) or instrumental activities of daily living (IADLs), multiple comorbidities or new disease diagnosis contributing to weakness, inability to attend outpatient services, patient being older and living alone, or history of more than one emergency room visit or urgent hospital admission in the past year 3, 6
  • The American Thoracic Society suggests that patients with functional limitations, multiple comorbidities, or a history of frequent hospitalizations may benefit from home health services, with a strength of evidence rated as moderate to high 3, 6

Specific Functional Assessment Documentation

  • The American College of Physicians recommends using standardized terminology with precise levels of assistance, such as independent, standby assist, minimal/moderate/maximal assistance, or total dependence, to document each ADL 7
  • The American College of Physicians suggests that documenting the patient's specific level of assistance for each basic ADL, such as bathing, dressing, toileting, transferring, continence, and feeding, is essential for determining the need for home health services 1, 7

Homebound Status Documentation

  • The Centers for Medicare and Medicaid Services requires explicit documentation of homebound status, including specific barriers such as inability to leave home without considerable and taxing effort, medical contraindication to leaving home, or cognitive impairment requiring a familiar environment for safety 1, 2

Skilled Service Orders

  • The American Thoracic Society recommends ordering physical therapy as the primary skilled service for weakness-related functional decline, with additional skilled services such as occupational therapy, skilled nursing, or home health aide services considered as needed 3, 4, 5, 6, 8
  • The American Thoracic Society suggests that physical therapy is specifically indicated for patients with primary problems of deconditioning, impaired mobility, or need for home adaptation, with a strength of evidence rated as high 3, 4, 5, 6, 8

Home Environment and Safety Assessment

  • The American College of Physicians recommends documenting specific environmental barriers and fall risk, including problems with stairs, bathtubs, rugs, lighting, and environmental modifications needed such as handrails or adequate lighting 1

Caregiver and Social Support Documentation

  • The American College of Physicians suggests listing all available caregivers by name and documenting their capacity to meet care needs, including caregiver limitations or inability to provide needed assistance, whether the patient lives alone, and caregiver anxiety or poor coping skills if present 1, 3

Plan of Care with Measurable Goals

  • The American College of Physicians recommends documenting specific, measurable functional goals, such as improving from maximal assistance to minimal assistance with bathing within 60 days, or ambulating 50 feet with a walker independently within 30 days 1
  • The American College of Physicians suggests that frequency and duration of each service type, such as physical therapy or occupational therapy, should be specified and regularly reassessed 1
