Olanzapine Use in Elderly Patients with Bipolar Disorder

Introduction to Olanzapine Therapy

  • The American College of Oncology recommends that olanzapine be prescribed to elderly patients with bipolar disorder with caution and at a reduced dose of 2.5-5 mg, considering the increased risk of side effects in this population 1, 2

Indications and Efficacy

  • Olanzapine is approved for the treatment of acute manic episodes in bipolar disorder I in adult patients, as recommended by the American Academy of Child and Adolescent Psychiatry 3, 4

Special Considerations in Elderly Patients

  • The recommended initial dose for elderly patients is 2.5-5 mg orally once daily, usually at night, as suggested by the American College of Oncology 1, 2
  • A dose reduction is necessary for elderly patients and those with impaired liver function, according to the American College of Oncology 2

Measures of Caution

  • Olanzapine should be used with caution in elderly patients due to the increased risk of side effects, as warned by the National Comprehensive Cancer Network 5, 1
  • There is a warning about the increased risk of death in elderly patients with psychosis related to dementia, as indicated by the FDA 5

Side Effects Requiring Special Attention in Elderly Patients

  • Common side effects include somnolence, fatigue, and sleep disturbances, as reported by the National Comprehensive Cancer Network 5, 1
  • Orthostatic hypotension is a concern, as noted by the American College of Oncology 1, 2
  • Metabolic effects, such as weight gain and hyperglycemia, can occur with long-term use, as cautioned by the National Comprehensive Cancer Network 5, 2
  • The risk of falls is increased, as warned by the American College of Oncology 2, 6
  • Rare but serious skin reactions, such as DRESS syndrome, can occur, as reported by the National Comprehensive Cancer Network 5

Drug Interactions

  • Caution should be exercised when combining olanzapine with metoclopramide, phenothiazines, or haloperidol due to the risk of excessive dopamine blockade, as advised by the National Comprehensive Cancer Network 5
  • Combining olanzapine with benzodiazepines increases the risk of excessive sedation and respiratory depression, as warned by the American College of Oncology 1, 7

Algorithm for Prescribing Olanzapine to Elderly Patients with Bipolar Disorder

  • Assess the risk-benefit ratio for the individual patient, as recommended by the American College of Oncology 1
  • Start with a low dose of 2.5 mg orally once daily and titrate slowly as needed, based on clinical response, as suggested by the American College of Oncology 1, 2
  • Regularly monitor metabolic parameters, orthostatic hypotension, sedative effects, and the risk of falls, as advised by the National Comprehensive Cancer Network and the American College of Oncology 5, 1, 2, 6
