Treatment of Type 2 Diabetes in Elderly Patients

Assessment and Treatment Approach

  • The American Diabetes Association suggests that an HbA1c of 6.8% indicates relatively good overall glycemic control despite elevated blood glucose levels, suggesting fluctuating glucose levels rather than consistently poor control 2
  • The American College of Cardiology recommends considering age-related factors, including risk of hypoglycemia, comorbidities, and simplified regimens to improve adherence in elderly patients with type 2 diabetes 3
  • SGLT2 inhibitors are preferred agents for elderly patients with type 2 diabetes due to once-daily dosing, low risk of hypoglycemia, and additional cardiovascular and renal benefits beyond glucose control, as recommended by the American Heart Association 1, 4
  • The American College of Cardiology also suggests that GLP-1 receptor agonists provide effective glucose control with low hypoglycemia risk and offer cardiovascular benefits and weight reduction, making them a suitable alternative option 1, 2, 4

Medication Selection and Monitoring

  • The American Diabetes Association recommends avoiding thiazolidinediones (TZDs) in elderly patients due to increased risk of heart failure, fluid retention, and fractures, and using sulfonylureas with caution due to higher hypoglycemia risk 4, 5
  • The American College of Cardiology suggests that if insulin is required, proper education on administration and hypoglycemia management is necessary, and consideration of the patient's renal function is crucial when selecting medications, particularly for metformin and SGLT2 inhibitors 6
  • The American Diabetes Association recommends scheduling follow-up within 1-3 months to assess response to new therapy, monitoring for medication-specific side effects, and reassessing HbA1c every 3-6 months to evaluate long-term glycemic control 1, 2
