Empiric Management of Early Lyme Disease (Erythema Migrans)

Adult First‑Line Antibiotic Regimens

The Infectious Diseases Society of America (IDSA) recommends three equally effective oral options for adults with clinically diagnosed erythema migrans.

Evidence of efficacy: Prospective randomized trials show complete response rates > 83 % at 30 months for all three agents, with no statistically significant differences. High‑quality evidence. 1

Antibiotic Regimens for Early Lyme Disease

Introduction to Treatment Regimens

  • The Infectious Diseases Society of America recommends doxycycline 100 mg twice daily for 10-21 days, amoxicillin 500 mg three times daily for 14-21 days, and cefuroxime axetil 500 mg twice daily for 14-21 days as first-line oral antibiotic regimens for treating early Lyme disease in adults 6, 7

Adult Dosing Regimens

  • The American College of Physicians suggests that doxycycline dosage is 100 mg orally twice daily, with a duration of 14 days (range 10-21 days) 6, 7, 8
  • The Infectious Diseases Society of America recommends amoxicillin dosage of 500 mg orally three times daily, with a duration of 14-21 days 6, 7, 8
  • The American Academy of Family Physicians suggests that cefuroxime axetil dosage is 500 mg orally twice daily, with a duration of 14-21 days 6, 7, 8

Pediatric Dosing Regimens

  • The American Academy of Pediatrics recommends doxycycline dosage of 4 mg/kg per day in 2 divided doses (maximum 100 mg per dose) for 14 days for children ≥8 years old 6, 8, 9
  • The American Academy of Pediatrics suggests amoxicillin dosage of 50 mg/kg per day in 3 divided doses (maximum 500 mg per dose) for 14 days for children <8 years old 6, 8, 9

Clinical Trial Evidence Supporting These Regimens

  • Multiple prospective randomized controlled trials demonstrated comparable efficacy across all three regimens, with no difference in complete response rates (83.9% vs 90.3% at 30 months, P > 0.2) 6, 7

Important Administration Considerations

  • Doxycycline should be taken with 8 ounces of fluid to reduce esophageal irritation and with food to minimize gastrointestinal intolerance, and patients must avoid sun exposure due to photosensitivity risk 7, 10
  • Doxycycline is relatively contraindicated in pregnant or lactating women and children under 8 years of age 6, 8

Common Pitfalls to Avoid

  • First-generation cephalosporins (e.g., cephalexin) are inactive against B. burgdorferi and should never be used 8, 9
  • Macrolide antibiotics (azithromycin, clarithromycin, erythromycin) are less effective than the three first-line regimens and should only be reserved for patients intolerant of all first-line options 6, 7, 8
  • Extending treatment beyond 21 days is not supported by evidence and does not improve outcomes 7

Errores Comunes a Evitar
