Adenovirus Infection Management in Children

General Principles

  • The American Academy of Pediatrics recommends supportive care with hydration, antipyretics, and respiratory support as needed for immunocompetent children with adenovirus infection, while cidofovir is reserved for life-threatening infections in severely ill or immunocompromised children 1
  • Supportive care is the cornerstone of management for the vast majority of children with adenovirus infection, with the infection typically resolving within 7-10 days without specific antiviral therapy 1

Immunocompetent Children with Mild-Moderate Disease

  • Adequate hydration is essential to maintain fluid balance in children with adenovirus infection 1
  • Antipyretics are recommended for fever management in children with adenovirus infection 1
  • Respiratory support, such as oxygen and nebulizers, may be necessary for children with adenovirus infection 1
  • Monitoring for clinical deterioration is crucial over the typical 7-10 day illness course 1

Infection Control Measures

  • Patients with adenovirus infection are infectious for 10-14 days from symptom onset 1, 2
  • Hand hygiene with soap and water is essential for preventing the spread of adenovirus infection 1
  • Dilute bleach solution is recommended for equipment and surface disinfection to prevent the spread of adenovirus infection 1
  • Isolation in hospitals and daycare settings is necessary to prevent outbreaks of adenovirus infection 1

Special Clinical Scenarios

  • The American Academy of Ophthalmology recommends symptomatic relief, such as artificial tears and topical antihistamines, for adenoviral conjunctivitis 2, 3, 4
  • Topical corticosteroids may be indicated for severe cases of adenoviral conjunctivitis with marked chemosis, lid swelling, epithelial sloughing, or membranous conjunctivitis 2, 3, 4

Severe or Life-Threatening Infections

  • Cidofovir is the treatment of choice for severe adenovirus infections, particularly in immunocompromised children or those with disseminated disease 1
  • The standard regimen for cidofovir is 5 mg/kg IV once weekly for 2 weeks, then every other week 1
  • Mandatory renal function monitoring is essential due to the considerable nephrotoxicity risk associated with cidofovir 1

Prognostic Factors

  • Adenovirus serotypes 3 and 7 have a worse prognosis 1
  • Age under 1 year with myocarditis is associated with a 5-year survival rate of only 66% with adenovirus versus 95% without 1
  • Immunocompromised status is associated with fatality rates exceeding 50% for untreated severe pneumonia or disseminated disease 1
