Immediate Management of Hemorrhaging Placenta Previa at 22 Weeks

Initial Assessment and Stabilization

  • The American College of Obstetricians and Gynecologists recommends hospitalizing immediately, avoiding digital examination, resuscitating aggressively with IV fluids and blood products as needed, and preparing for potential delivery only for maternal indications such as life-threatening hemorrhage—not for fetal benefit, as cesarean delivery before 22 weeks is appropriate only for maternal indications 1
  • Avoid digital vaginal examination until placenta previa has been excluded or confirmed by ultrasound, as this can trigger catastrophic hemorrhage 2

Diagnostic Evaluation

  • The American College of Radiology recommends performing transabdominal ultrasound initially, followed by transvaginal ultrasound for precise diagnosis—transvaginal approach is both safe and accurate for placenta previa 2
  • Confirm placental location relative to internal cervical os and assess for placenta accreta spectrum disorder, particularly critical given any prior uterine surgery 4

Critical Decision Point: Delivery Considerations at 22 Weeks

  • The American College of Obstetricians and Gynecologists states that at 22 weeks gestation, delivery decisions must prioritize maternal survival exclusively, as cesarean delivery carries significant maternal morbidity without proven fetal benefit 1
  • Life-threatening hemorrhage unresponsive to resuscitation is a maternal indication for delivery 1

Ongoing Inpatient Management

  • The American College of Obstetricians and Gynecologists recommends maintaining strict bed rest during active hemorrhage and serial hemoglobin monitoring and transfusion to maintain hematocrit ≥30% is not cited, however, transfusing packed red blood cells to maintain hemoglobin and prevent maternal decompensation is a common practice, for massive hemorrhage, use massive transfusion protocol with 1:1:1 to 1:2:4 ratio of packed red blood cells:fresh frozen plasma:platelets 4

Counseling Considerations at Periviable Gestational Age

  • Provide accurate information about extremely poor neonatal outcomes at 22 weeks gestation and explain that maternal safety is the priority and cesarean delivery at this gestational age significantly increases maternal morbidity without proven fetal benefit 1
