Antibiotic Regimen of Choice for Tuboovarian Abscess

Initial Parenteral Therapy Options

  • The Centers for Disease Control and Prevention recommends parenteral clindamycin 900 mg IV every 8 hours plus gentamicin for the treatment of tuboovarian abscess, with gentamicin having a loading dose of 2 mg/kg followed by 1.5 mg/kg every 8 hours or once daily 1, 2
  • Clindamycin 900 mg IV every 8 hours provides essential anaerobic coverage for TOA treatment 3
  • Gentamicin loading dose IV or IM (2 mg/kg) followed by maintenance dose (1.5 mg/kg) every 8 hours or once daily is recommended for TOA treatment 1, 4
  • The clindamycin-gentamicin regimen provides superior anaerobic coverage essential for TOA treatment 2, 5
  • Cefotetan 2 g IV every 12 hours OR Cefoxitin 2 g IV every 6 hours PLUS doxycycline 100 mg orally or IV every 12 hours is an alternative regimen for TOA treatment 1, 4, 6

Transition to Oral Therapy

  • Parenteral therapy should continue for at least 24 hours after clinical improvement, then transition to oral therapy to complete a total of 14 days 1, 4
  • Oral clindamycin 450 mg four times daily is the preferred oral therapy for TOA 1, 2, 3
  • Alternative oral therapy is doxycycline 100 mg orally twice daily plus metronidazole 500 mg orally twice daily 4, 5

Special Considerations for TOA

  • At least 24 hours of direct inpatient observation is recommended for patients with TOA before transitioning to outpatient therapy 5, 7
  • Inadequate anaerobic coverage, such as using doxycycline alone without anaerobic coverage, is insufficient for TOA treatment 1, 2
  • Completing the full 14-day course of antibiotic therapy is essential even after clinical improvement 4, 5
