Management of Tunneled Catheter Infections
Classification and Management of Tunneled Catheter Infections
- The Kidney Disease Outcomes Quality Initiative (KDOQI) recommends that tunneled catheter infections be managed based on the type of infection, with exit site infections treated with topical antibiotics and appropriate local care, while catheter-related bacteremia requires parenteral antibiotics and catheter removal if the patient remains symptomatic for more than 36 hours or is clinically unstable 1, 2
- Exit site infections are characterized by redness, crusting, and exudate at the exit site without systemic symptoms and negative blood cultures, and should be treated with topical antibiotics and proper local care 1, 2
- For catheter-related bacteremia, the KDOQI recommends initiating parenteral antibiotics appropriate for suspected organisms, and removing the catheter if the patient remains symptomatic for more than 36 hours, is clinically unstable, or has tunnel tract involvement 1, 2
Antibiotic Selection and Duration
- Initial empiric therapy should cover both Gram-positive and Gram-negative organisms, according to the KDOQI guidelines 2
- For uncomplicated infections when salvaging the catheter, antibiotic lock therapy should be used for 2 weeks along with standard systemic therapy, as recommended by the Infectious Diseases Society of America 3
- For catheter-related bacteremia, a minimum of 3 weeks of systemic antibiotic therapy is recommended by the KDOQI 1, 2
Catheter Salvage vs. Removal Decision Algorithm
- The KDOQI recommends removing the catheter immediately if the patient is clinically unstable, has tunnel tract infection or port abscess, remains symptomatic after 36 hours of appropriate antibiotic therapy, or has complicated infection with septic thrombosis, endocarditis, or metastatic seeding 1, 2, 3
- Catheter salvage with guidewire exchange should be considered if the patient is clinically stable, has no exit site or tunnel tract involvement, and responds to initial antibiotic therapy, as recommended by the KDOQI 1, 2
