Glimepiride Dosing Recommendations for Special Populations and Chronic Kidney Disease

Elderly Patients

  • Initiate glimepiride at 1 mg once daily and titrate slowly in older adults to reduce hypoglycemia risk. 1

Patients with Renal Impairment

  • Begin treatment with glimepiride at 1 mg once daily and increase the dose cautiously in individuals with reduced kidney function, aiming to minimize hypoglycemia. 1, 2

Advanced Chronic Kidney Disease (CKD Stages 3b–5)

  • In patients with advanced CKD, start glimepiride conservatively (e.g., 1 mg daily) and titrate gradually, recognizing the heightened susceptibility to hypoglycemia. 1, 2

Sulfonylurea Choice in CKD

  • Glyburide (glibenclamide) is contraindicated in CKD, whereas glimepiride may be used with caution after careful dose adjustment. 1, 2
