Rheumatoid Arthritis Treatment Guidelines

Initial Treatment Strategy

  • The American College of Rheumatology recommends initiating methotrexate at 15 mg/week orally with folic acid 1 mg daily in most patients without contraindications 1, 2, 3
  • Lower doses are required in elderly patients and those with chronic kidney disease 1, 2
  • Methotrexate has the most favorable efficacy/toxicity balance compared to other conventional DMARDs and represents the anchor treatment for RA 1, 2
  • The TEAR trial demonstrated no advantage of initial combination therapy over MTX monotherapy with step-up at 6 months for inadequate response 1, 2, 3

Treatment Targets and Monitoring Timeline

  • The European League Against Rheumatism recommends targeting low disease activity (SDAI ≤11 or CDAI ≤10) at 3 months 5, 3, 4
  • Monitor disease activity every 1-3 months during active disease 6
  • If no improvement by 3 months, adjust therapy immediately 4, 6
  • More than 75% of patients achieving low disease activity at 3 months will be in remission at 1 year 3

Escalation Strategy for Inadequate Response

  • Optimize methotrexate dosing by increasing to 20-25 mg weekly or maximal tolerated dose 5, 1
  • Switch to subcutaneous administration if oral MTX inadequate 5
  • Add sulfasalazine + hydroxychloroquine (triple-DMARD therapy) if SDAI remains 11-26 (or CDAI 10-22) despite optimized MTX 5, 4
  • Add biologic DMARD to methotrexate when poor prognostic factors present or SDAI >26 (CDAI >22) 5, 4

Biologic DMARDs

  • First-line biologic options include TNF inhibitors (adalimumab, etanercept, infliximab, certolizumab, golimumab) 4
  • Abatacept (CTLA4-Ig, T-cell costimulation blocker) is also a first-line biologic option 4
  • Tocilizumab (anti-IL-6 receptor antibody) is a first-line biologic option 5, 4
  • Rituximab (anti-CD20) is indicated after inadequate response to at least one TNF inhibitor 5, 4

Treatment De-escalation in Sustained Remission

  • Consider tapering biologic DMARDs (dose reduction or interval prolongation) after achieving sustained remission (≥1 year) 4
  • Taper and discontinue prednisone first 5
  • Cautious reduction of conventional DMARD dose may be considered as shared decision with patient 4
  • Continue current regimen if remission maintained 5
