Management of Refractory Vomiting in Cancer Patients

  • The American Society of Clinical Oncology recommends that NK-1 receptor antagonists have documented antiemetic activity in patients who did not achieve complete control with 5-HT3 antagonists 3

Non-Chemotherapy Etiologies

  • Malignant bowel obstruction is common in advanced colorectal and ovarian cancer, and can cause refractory vomiting in cancer patients 1, 2
  • Constipation is a frequent cause of refractory vomiting, particularly in patients on opioids or anticholinergic medications 1, 2
  • Hypercalcemia is a common metabolic cause of refractory vomiting in cancer patients 1, 2
  • Brain metastases can trigger central vomiting mechanisms and cause refractory vomiting in cancer patients 1, 2

Critical Management Considerations

  • The National Comprehensive Cancer Network suggests that antiemetic agents targeting different neuroreceptors behave synergistically when used in combination 1, 2
  • Withholding more effective antiemetics for later use is counterproductive, as antiemetics are most effective when used prophylactically 3
