Treatment Approaches for Cholestasis
Diagnostic Approach
- The American Association for the Study of Liver Diseases recommends differentiating between intrahepatic and extrahepatic cholestasis using abdominal ultrasound as the first-line imaging study 1, 2
- For suspected extrahepatic obstruction, the European Society of Gastrointestinal Endoscopy suggests that ERCP is the gold standard for visualization and treatment, while MRCP or endoscopic ultrasound (EUS) should be performed first to avoid unnecessary ERCP and its complications 1, 3
Treatment of Obstructive Cholestasis
- The American College of Gastroenterology recommends removal of the obstruction through interventional procedures, with endoscopic intervention for relevant strictures and endoscopic sphincterotomy and stone extraction for bile duct stones 1, 3, 4, 5
- The European Association for the Study of the Liver suggests management of complications, including acute bacterial cholangitis with antibiotics and subsequent biliary decompression, pruritus with pharmacological treatment, and portal hypertension according to general guidelines for advanced chronic liver disease 1, 4
Treatment of Viral Cholestasis
- The World Health Organization recommends treatment of the underlying viral infection with antiviral therapy appropriate for the specific viral pathogen, as well as supportive care while the infection resolves 6, 7
- The American Association for the Study of Liver Diseases suggests management of pruritus with cholestyramine, rifampicin, and naltrexone, and correction of fat-soluble vitamin deficiencies 5, 8
Special Considerations
- The American College of Obstetricians and Gynecologists recommends ursodeoxycholic acid (UDCA) for intrahepatic cholestasis of pregnancy, as well as vitamin K supplementation when prothrombin time is prolonged 5
- The European Association for the Study of the Liver suggests consideration of immunosuppressive therapy for PSC-AIH overlap and liver biopsy with expert clinicopathological assessment for PBC-AIH overlap 4, 9, 10
