Conservative Management for Moderate Degenerative Changes in the Knee

First-Line Conservative Approaches

  • Structured physical therapy with focus on strengthening exercises for the quadriceps and hip muscles is the cornerstone of conservative management for knee osteoarthritis, as recommended by the British Medical Journal 1
  • Even modest weight reduction can significantly improve symptoms and function in patients with knee osteoarthritis, according to the American Academy of Orthopaedic Surgeons 2

Pain Management

  • Topical NSAIDs can provide localized pain relief with fewer systemic side effects, as reported by the British Medical Journal 1
  • Corticosteroid injections can provide short-term pain relief (typically 2-3 months) for moderate to severe pain, as stated by the American Academy of Orthopaedic Surgeons 2

Assistive Devices and Bracing

  • A cane used in the hand opposite to the affected knee can reduce medial compartment loading and improve stability, as recommended by the American Academy of Orthopaedic Surgeons 2
  • Unloader braces may help redistribute load away from the affected medial compartment, according to the American Academy of Physical Medicine and Rehabilitation 3

Patient Education

  • Educate patients on the nature of degenerative knee disease and realistic expectations from conservative management, as suggested by the British Medical Journal 1
  • Explain that knee replacement is the only definitive therapy but should be reserved for severe disease after conservative management has been unsuccessful, as stated by the British Medical Journal 1

Important Considerations and Pitfalls

  • Avoid recommending arthroscopic surgery, as evidence strongly shows it does not provide meaningful benefits over conservative management for degenerative knee disease, as reported by the British Medical Journal 1, 4, 5

Follow-up and Progression

  • If symptoms worsen despite 3-6 months of appropriate conservative management, consider referral for surgical evaluation, as recommended by the American Academy of Orthopaedic Surgeons 2
