Diagnostic Approaches for Duplex Collecting Systems

Primary Recommendations

  • The American Academy of Pediatrics recommends Tc-99m MAG3 renal scintigraphy as the best modality for determining differential function in a duplex collecting system of the kidney, due to its ability to directly measure split renal function through parenchymal uptake during the first pass from circulation into the kidney 1, 2
  • MAG3 scintigraphy provides quantitative differential function by calculating the relative contribution of each kidney (or each moiety in a duplex system) to overall renal function, with superior image quality even in patients with low glomerular filtration rate 1, 2
  • The American College of Radiology suggests that MAG3 can distinguish between dilated obstructed versus non-obstructed systems using diuretic administration, making it a valuable tool for assessing drainage patterns 1, 2, 3

Alternative Modalities

  • Functional MR Urography (MRU) is a reliable alternative to MAG3 when radiation avoidance is critical, particularly in pediatric patients, and can provide detailed morphologic characterization of both moieties and their drainage patterns 1, 2, 4
  • The American College of Radiology notes that MRU has significant practical limitations, including the need for sedation or anesthesia in younger children, higher cost, and limited availability compared to nuclear medicine 1, 2

Role of DMSA Scintigraphy

  • DMSA is ideal for cortical imaging and can calculate differential function, but provides no drainage information, making it less useful than MAG3 for duplex systems where obstruction assessment is often needed 5, 6

Clinical Decision Algorithm

  • The American Academy of Pediatrics recommends a clinical decision algorithm for duplex collecting systems, where MAG3 renal scintigraphy is the first-line choice for all patients requiring differential function assessment, and MRU is considered when radiation minimization is a priority or complex anatomy requires detailed morphologic mapping 1, 2, 4

Common Pitfalls to Avoid

  • The American College of Radiology advises against relying on ultrasound alone for functional assessment, as it provides excellent anatomic detail but cannot quantify differential function 5, 6
  • Ensuring adequate hydration before MAG3 study is crucial, as dehydration can produce false-positive obstruction patterns 1, 2

Radiation Considerations

  • MAG3 has a low effective radiation dose, with over 95% of radiotracer cleared within 3 hours in patients with normal kidney function, justifying its use when critical functional information is needed 1, 2
